Your neck contains four tiny, pea-sized glands called the parathyroid glands. Even though they are near your thyroid, they are completely separate. Their main job is to release a hormone that keeps your blood calcium and phosphorus at healthy, balanced levels.
When you have hypoparathyroidism, these tiny glands don't produce enough of that hormone. Because calcium is essential for your nerves and muscles to work correctly, having too little of it can make you feel unwell and cause your muscles to twitch or cramp.
